All extended sessions will be prorated based on the Follow-up Session rate, and this extension will not be covered by insurance, so must be paid by client at time of services.
At this time, I am accepting Aetna, Christus Health & Blue Cross/Blue Shield. If I currently do not accept your insurance, I will provide you with a detailed session receipt that you may file with your insurance company if they will reimburse out of network providers.
Please verify coverage with your provider.
Our policy requires you to give a 24-hour notice if you need to cancel or reschedule an appointment. The Follow-up session rate will be charged if this notice is not given. This is not covered by your insurance.
